describes the chronology of the issues that are plaguing Toyota
today.
The article
describes how evidence had been mounting for years that Toyota
cars could suddenly speed up, which was a factor suspected
in crashes that has caused death. Toyota indicated that the
problem was floor mats pinning the gas pedal; however, now
two Toyota men revealed they knew of a problem in its gas
pedals.
The Toyota Production System (TPS) has been
held upon a pedistal for many years. With this TPS system
and benchmark lean practicies, how could such problems occur

to do with an out-of-control point(s) in an control chart.

As improvement
practitioners, we often plot a control chart and find one
or more out of control conditions. The question is what to
do next, right?
Rick's
Rules:
- Out
of control indications indicate a high probability of finding
a special cause, not a guarantee that there is one.
- All
real data should stay in the chart, only remove wrong values.
- True
special cause events should be noted on the chart with a
label or a stage, not deleted.

action or inaction, which is independent of the person
compiling the information.
This
basic right-behavior objective is like an inspection
gage Measurement Systems Analysis (MSA), which ensures
that inspectors can adequately determine whether a manufactured
component should be accepted or rejected. Because of
this performance-reporting need, it seems that management
and practitioners—from a conceptual MSA point
of view—would be assessing the health of current
scorecard and metric reporting systems.
However,
this doesn’t seem to be occurring. The question
is: Why don’t we examine business metrics
and process capability indexes reporting from an MSA
point of view with the same level of intensity that
we do for product quality metrics?

balanced scorecard is a commonly used vehicle that is
to align organizational-chart work efforts to executive-determined
strategies and goals. With this approach, strategic
planning could be considered as step one in this overall
business-management-system process.
Consider
now the true needs of a business. For success, it is
important that organizations move toward achievement
of the three Rs of business—everyone doing the
right things, doing them right, and at the right time.
